Form of consent and regulations
Every club member must be informed about all our regulations.
We focus in children and we take care about offering a happy environment free of pressure. We understand that children are children and they want to play and share with friends.
I give my consent for my child to participate in Olfos sports academy sports programs. I understand all risks that I will have in exercise sessions, taking fully responsibility if any injury or life risk situations may have before, during or after these activities.
I was recommended by Olfos sports Academy to present a written clearance from my primary doctor, before be part in any of their sports programs just for my safety.
This physical activity clearance is valid for a maximum of 12 months from the date it is completed and becomes invalid if my condition changes. I also acknowledge that olfos sports academy may retain a copy of this form for records.
I understand that Olfos sports academy don’t do refund for any kind of programs, however you can cancel your membership anytime you need. Please in order to cancel your membership, you must send an email to olfossportsacademy@gmail.com with the name of your child and the name of the parents.
Sessions could be suspended because of weather conditions, wet fields or thunderstorms threat. Club has no obligation to recover these sessions.
Participants can’t wear jewelry, chains, earrings, necklace jewelry or other objects that could be dangerous during physical activities. Also, they can’t train chewing gum or eating some kind of candy.
Our club highly recommends the proper use of protection items such as shin guards, mouth guards and the proper shoes for practicing sports.
Its totally prohibited to yell at the children or give them instructions during a training session or game. This will affect your child performance and other children too. Those parents or adults that are in the field doing any negative manifestation or interfering with a happy sports festivity, will be removed from the field. If it necessary, we will call the police if we have to.
Any violent actions or use of bad words performed by parents, relatives, friends or even our own players, will be separated of the club. Our directory will make a decision if this player or person that performed this violent act or use of inappropriate language can be back or not to the club.
Our main goal is that children paly their favorite game in a safe and happy environment. We believe that the best way to develop them is in a environment free of pressure.
Consent to release photo / image
I hereby grant permission to olfos sports academy representatives, to take and use photographs, videos and / or digital images of my child for use on Olfos sports academy web site and social media profiles and in news releases and / or promotional materials as follows:
Printed publications or materials, electronic publications, or web sites. I agree that my child’s name and identity may be revealed in descriptive text or commentary in connection with the image (s).
I authorize the use of these images without compensation to me. All negatives, prints digital reproductions and shall be the property of Olfos sports academy.
